You can print the file directly from Acrobat Reader in either Letter or A4 formats. You can fill the printed page with 4 bookmarks if you select "Tile Large Page" on Page Scaling on the Print dialogue box.

For the best result: use good quality inkjet cards, set your printer to the best quality and highest resolution.

Use a scalpel or paper trimmer to cut out the book marks. Use a hole puncher to punch a hole on the top.
